Your industry and location will alter what your requirements are.
You have a couple of different options to ensure that you get accurate answers to your questions.
You can go to your city hall and local chamber of commerce and ask to speak with the department / people responsible for answering these questions. This may end up being a lengthier way, but will save you money.
If you are in a hurry or don't want to run around speaking with various people to get all your questions answered and don't mind spending a little money, go to BizFilings.com. There you will find a tab, "Products". The menu under this tab includes Corporate Compliance Services, DBAs, Licenses, EINs, and Incorporation - amongst other things. I found it very useful.
