California minimum $800 franchise Tax

I would like to know about California FTB minimum $800 franchise tax. My question is

- Form number to attach with check.
- Address to send form and check
- Last date to pay

Sorry, I'm not from CA but you really neeed to seek a good CPA. Ask around who has a good CPA to meet your needs. They don't need to be a big firm, just someone to meet your needs and guide you.

Just in case anyone in the future gets directed to this page through Google, as I did, this is an easily answerable question:

The $800 is sent with Form FTB 3522 available at ftb.ca.gov

It is sent to:
Franchise Tax Board
PO Box 942857
Sacramento, CA 94257-0631

It is due on April 15th, except for your first year in business. That first year it is due the 15th day of the 4th month after you incorporate. When you count to the "fourth month" make sure you start with the incorporation month. For example, if you incorporated in May, your $800 would be due August 15th.

Really now, it doesn't take a CPA or a link to the IRS (?) to answer this!

It seems like if I start the LLC in Jan, I would have to pay USD 1600 in April: USD 800 for the first year in business, and USD 800 for the annual tax due on April 15th.

Is it my miscalculation or is it really best to start the business right after April 15th, say, in the beginning of May?
