Procedure for filing for Registered Agent in MA

Hi,
I had originally filed with a Registered agent my S-corp. I wish to file with the Secretary of State - myself as the registered agent since my business is in my city.

I am in MA and wanted to know the form and details on the filing. If anyone can help me with the process it would be helpful. My deadline is Sept 1 after which I will get billed by my Registered Agent (I need to show them a prof of filing)
