Help with filling out Assumed Name Certificate form

Hi, I live in New York and plan to form a sole proprietorship. I was told to file an assumed name certificate form but I'm having some problems filling it out.


here is the form:

1. Name of Entity? would this be my name? or the business name that I'm planning to use?

1a. Foreign Entities only?

2. New York law formed or authorized under? should I check "Other" and fill in "Sole Proprietorship? I only plan to form a sole proprietorship business.

3. Assumed Name? again, my name or business name?

If anyone could help me, I would really appreciate it.

I would suggest you find a local SCORE or SBDC office in your area. These are free to use and there to help you. They can help you fill out the form as well as with other aspects of your business. They are free and there to help you -

You'll need form x201 and you need to file it in your county clerk's office.
I already filed mine on friday.. which only took about 20 minutes.
You can buy the form from a stationary shop near the county clerk's office.

I can try to help you with this.

1. Name of entity - Put the name you have chosen for your company.

1a. Foreign entities only - Do not put anything here. This is only for foreign companies that want to do business in NY.

2. New York law formed or authorized under - Check BCL or Sole Proprietorship under "other" (ask someone from NY about this).

3. Assumed name - Put the name of your company again.

4. Principle place of business - Put your business address.

5. Counties in which .... - Check all

I live in Los Angeles, CA and here we need to fill up a Fictitious Business Name form also known as DBA (Doing Business As). It looks like the requirements for registration of a business name in CA and NY are pretty similar. But in CA you can check online whether the name you've picked for your company is still available. See if you can do this online in NY as well - it will save you time.

Will you have a website for your business? If so, I suggest you check if there is a domain name available similar/identical to the name of your company. When I registered my company last month, I made sure that the same domain name hasn't been taken by somebody else.

he's using the wrong form, that form is for corporations. please use form x201 for sol proprietorship
